At Health Choice Generations Utah HMO SNP, we are committed to a collaborative approach with physicians, hospitals and all other providers in the medical communities we serve.

We believe that those who provide care should be the leaders in creating and constructing new, better and less invasive mechanisms for the delivery of the care they provide. We are provider-owned and we understand both the rewards and difficulties of managed care and health plan/provider relationships.

We believe our members deserve the highest quality medical care while being treated with both compassion and respect. Assisting our providers so that you can devote your time to providing quality patient care is one of our highest priorities. Our commitment to you is to support the doctor-patient relationship by streamlining the delivery of care.

What We Offer Our Physician Network

Health Choice Generations brings the expertise and road maps necessary to understand, participate in and maximize the value of the sweeping changes affecting the delivery of health care.

We offer real-time tools, technology and up-to-date information to our physicians and providers. We will assist and offer guidance to physicians and hospitals for the purpose of building partnerships, patient-centered medical homes and other entities that will maximize quality and reward performance.

How We Assist Your Practice

• The Health Choice Generations Provider Portal is designed with you in mind. Included in this site, and accessed through a secure portal, is patient data such as claims history and prior authorizations.
• We work closely with your team to streamline and expedite prior authorization by minimizing the number of procedures requiring prior authorization. Many of the items on our abbreviated prior authorization list ask for notification only.
• Customer service is the fabric our organization. Our internal infrastructure is designed to provide your patients speedy access to customer service representatives and to expedite claims processing.

How to join Health Choice Generations Utah

Please complete a Request for Participation Form, including supporting documentation, if you are interested in becoming a contracted provider with Health Choice Generations. Should your request be approved and dependent upon your specialty and location, our all product agreement may also include participation in our Medicaid health plan, Health Choice Utah.

Once the Request for Participation Form is received by the Health Choice Generations Provider Services Department, we will evaluate your request based upon business needs, location, and Provider adequacy.

If your request is approved, then the Provider Services Department will contact you to initiate the Contracting and Credentialing process. Health Choice Generations credentials providers according to nationally recognized standards. We do not discriminate against age, color, creed, disability, national origin, race, religion, sex, or sexual orientation. Credentialing generally takes about 60 days from our receipt of a complete credentialing application and up-to-date attachments.
