Customer Service

At Health Choice Generations Utah HMO D-SNP, members are our number one priority. Our mission is to treat you with the utmost respect and dignity while providing you safe, effective, quality healthcare.

One way to accomplish this is to provide members with outstanding customer service. Health Choice Generations has formed a team of knowledgeable, experienced Customer Service representatives. Customer Service’s sole purpose is to help resolve any questions or problems.

A few examples the Health Choice Generations Customer Service team can assist with:

  • Explaining benefits offered by the plan and answering general questions
  • Locating primary doctors, specialists, or pharmacies in your area
  • Updating demographic information on your member file
  • Ordering new or replacement Member ID cards

Our Customer Service Department is open 8 a.m. – 8 p.m., 5 days a week (April – September) or 7 days a week (October – March) and can be reached by calling 1-844-457-8943. TTY users should call 711.
